Home support
Services of the home support program are intended for people living with a disability that permanently or temporarily confines them to their home and whose health condition requires certain specific care or assistance. These services are offered to seniors or people of all ages who are recovering or suffering from a chronic disease, people living with a physical or intellectual disability, and users who need support in palliative care.
The goal is to keep people with decreasing autonomy, or living with a limitation, in their natural living environment as long as possible, to avoid hospitalizations, or at least to reduce their duration and the associated risks of decrease in autonomy. The home support team is made up of nurses, social workers, home aids and, when necessary, occupational therapist, physiotherapist, nutritionist or other intervenors.
